I confirm that the information I have given provides a true and complete representation (to the best of my knowledge) of my medical history and current condition.  I declare that I am physically and mentally capable of participating safely.

I understand that the activities associated with the experience that I am attending have risks and are possibly dangerous.  I understand that although Adventure Awaits will teach me how to manage these risks and has completed appropriate procedures and risk assessments to help minimise and manage them, Adventure Awaits can never remove them completely.  I confirm that I accept responsibility for my own actions and involvement when participating and confirm that I will not act in an inappropriate way that can lead to harm to other participants or the instructors. 

I authorise Adventre Awaits or appropriate first aid or medically qualified staff to give medical treatment during the activity during an emergency and to contact my emergency contact in the event of hospitalisation.
